WRITTEN Opinion entered by the Honorable Thomas M. Durkin on 4/10/2013: Plaintiff's motion for leave to proceed in forma pauperis 3 is granted. The Court authorizes and orders Stateville Correctional Center officials to deduct $7.00 from Plaintiffs account, and to continue making monthly deductions in accordance with this order. The Clerk shall send a copy of this order to the trust fund officer at the Big Muddy River Correctional Center. The Clerk shall also issue summons for servi ce of the complaint on Defendant Officer Guy and send Plaintiff a Magistrate Judge Consent Form and Instructions for Submitting Documents along with a copy of this order.
