OPINION, MEMORANDUM AND: HEREBY ORDERED that plaintiff's motions to withdraw the supplemental complaints filed in this action [Doc. # 13 , # 14 and # 16 ] are GRANTED. IT IS FURTHER ORDERED that plaintiff's motion to proceed in forma pauperis [Doc. # 7 ] is DENIED and this case is DISMISSED pursuant to 28 U.S.C. § 1915(g). MEMORANDUM AND ORDER: IT IS HEREBY ORDERED that the Clerk shall mail to plaintiff a copy of the Court's form Prisoner Civil Rights Complaint. IT IS FURTHER ORDERED that the Clerk shall mail to plaintiff a copy of the Court's form Motion t o Proceed in Forma Pauperis- Prisoner Cases. IT IS FURTHER ORDERED that plaintiff shall file an amended complaint on the Court-provided form in compliance with this Court's Memorandum and Order within thirty (30) days of the date of this Order. IT IS FURTHER ORDERED that plaintiff shall either pay the $400 filing fee or submit a motion to proceed in forma pauperis, along with a statement regarding his claims of imminent danger, within thirty (30) days of the date of this Memorandum and Order. IT IS FURTHER ORDERED that if plaintiff fails to comply with thisMemorandum and Order, the Court will dismiss this action without prejudice. Response to Court due by 9/21/2014.
