Filing 6 MEMORANDUM AND ORDER IT IS HEREBY ORDERED that plaintiff's motion to proceed in forma pauperis [Doc. 2 ] is GRANTED. IT IS FURTHER ORDERED that plaintiff shall pay an initial partial filing fee of $1.50 within thirty (30) days from the da te of this Order. Plaintiff is instructed to make his remittance payable to "Clerk, United States District Court," and to include upon it: (1) his name; (2) his prison registration number; (3) the case number; and (4) that the remittance is for an original proceeding. FURTHER ORDERED that plaintiff shall submit an amended complaint, in accordance with the instructions set forth in this Memorandum and Order, no later than thirty (30) days from the date of this Memorandum and Order, a nd his failure to do so will result in the dismissal of this action without prejudice and without further notice. FURTHER ORDERED that the Court shall mail to plaintiff a blankform for the filing of a prisoner civil rights complaint. Plaintiff may requestadditional forms from the Clerk, as needed.
