Names the individual(s) personally responsible for his confinement at the Staton Correctional Facility and the conditions present at such facility. 2. Specifically describes how each defendant named in the amended complaint violated his constitutional rights regarding his confinement at Staton, as further set out in order. To aid the plaintiff in filing an amended complaint in response to the directives of this order, the Clerk is DIRECTED to provide him with a copy of the form used by prisoners to file 1983 complaints, and the plaintiff shall use this form in filing his amended complaint. The amended complaint will supersede the initial complaint. Thus, the court will consider only the claim(s) set forth in the amended complaint and the defendant(s) named in the amended complaint. The plaintiff is advised that the amended complaint should not repeat claims presented against defendants in other lawsuits as such claims would be subject to summary dismissal as malicious under either 28 U.S.C. 1915(e)(2)(B)(i) or 28 U.S.C. 1915A(b)(1).
