If neither party requests reassignment, the parties should complete the Court's form Consent to the Exercise of Jurisdiction by a United States Magistrate Judge and return it to the Court by December 6, 2024. The Clerk is DIRECTED to send to each party a Request for Reassignment to a United States District Judge form and the Court's Consent to the Exercise of Jurisdiction by a United States Magistrate Judge form.
