ORDER the Clerk of Court must strike the 2254 Petition 1 and Application to Proceed In Forma Pauperis 2 that were filed by Douglas T. Kakuda. Petitioner has 15 days from the date this Order is filed to file an amended 2254 Petition and an Applica tion to Proceed In Forma Pauperis, both of which must be signed by Petitioner personally or an attorney who is permitted to practice in this District Court. If Petitioner fails to comply, the Clerk of Court must enter a judgment of dismissal without prejudice. The Clerk of Court must mail Petitioner a court-approved form for filing an Application to Proceed In Forma Pauperis (Habeas) and the court-approved form for filing a "Petition Under 28:2254 for Writ of Habeas Corpus by a Person in State Custody (Non-Death Penalty)".
