Plaintiff and Defendant are advised that, to encourage the just and speedy determination of civil action filed in this District, the parties may consent to have a U.S. Magistrate Judge render final judgment. The Clerk shall serve Plaintiff and Defendant with a copy of the Statement of Consent to Proceed before a United States Magistrate Judge form (Form CV-11D) along with this Order. By no later than April 10, 2019, Plaintiff and Defendant shall file a completed Form CV-11D with the Clerk indicating whether they do or do not consent to Magistrate Judge Jurisdiction.
