MINUTE ORDER In response to the Order Directing Plaintiff to Cure Deficiencies (ECF No. #3 )1 Plaintiff has submitted on the wrong form a Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 in a Habeas Corpus Action (ECF No. #4 ). Plaintiff will be given another opportunity to cure the deficiencies on or before January 18, 2019, by filing on the proper form a Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 along with a current, certified copy of his inmate trust fund account statement and a signed authorization to calculate and disburse filing fee payments. Plaintiff is reminded that the action will be dismissed without further notice if he fails to cure all of the deficiencies within the time allowed. The clerk of the court is directed to mail to Plaintiff, together with a copy of this Minute Order, a blank copy of the following form: Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915.
