The majority of the form Complaint has been left blank, including the dates on which the Administrative Law Judge and Appeals Council issued their respective decisions. See Doc. #1 at 2. Without that information, the Court is unable to ascertain whether plaintiff has timely filed this action.Accordingly, on or before January 31, 2019, plaintiff shall file an amended complaint which includes the dates on which the Administrative Law Judge and Appeals Council issued their respective decisions. Alternatively, on or before January 31, 2019, plaintiff may file any supplemental information (i.e., a copy of the Appeals Council decision), which cures the deficiencies identified in this order.Failure to comply with this order may result in the dismissal of plaintiff's Complaint. January 10, 2019 Opinion or Order Filing 8 ORDER granting #2 Motion for Leave to Proceed in forma pauperis. Plaintiff has filed a motion seeking to proceed without payment of fees and costs, which motion includes a sworn statement as to plaintiff's current financial circumstances. See Doc. #2 . Plaintiff has completed the required information. He asserts that he is unable to afford to pay fees and costs, having no income and minimal assets. See id. at 3-4. Plaintiff also states that he receives $219 per month in welfare benefits and $192 per month in SNAP benefits. See id. at 3, 5. At this stage, such allegations are sufficient to establish that plaintiff is unable to pay the ordinary filing fees required by the Court. 28 U.S.C. 1915(a)(1).
