Filing 7 ORDER. The self-represented plaintiff has filed #2 Motion for Leave to Proceed in forma pauperis, which seeks to proceed in this matter without the payment of fees or costs. See generally Doc. #2 . Attached to plaintiff's motion is a sworn statement as to plaintiff's current financial circumstances. See id. at 2-5. That sworn statement, however, is incomplete, leaving the Court unable to fully assess plaintiff's financial circumstances. Plaintiff states that he receives $2,640 per month in social security, disability or workers' compensation benefits, see id. at 3, but he provides no information concerning his monthly obligations, having crossed out that entire section of the financial statement, see id. at 4-5.Accordingly, on or before the close of business on February 11, 2019, plaintiff shall file an Amended Motion for Leave to Proceed in forma pauperis, which must include all of the required information, including any of plaintiff's monthly obligations. Plaintiff may indicate that his obligations are "$0" or that a particular question does not apply to his situation, but he may not leave significant sections of the affidavit form blank or crossed out. Alternatively, on or before the close of business on February 11, 2019, plaintiff may pay the required filing fee. Failure to timely file an Amended Motion for Leave to Proceed in forma pauperis or pay the filing fee may result in the dismissal of this action. See, e.g., Schwarz v. I.R.S., 998 F. Supp. 201, 202 (N.D.N.Y. 1998) (denying application to proceed in forma pauperis as incomplete where plaintiff failed to complete application).It is so ordered. Signed by Judge Sarah A. L.
