Filing 11 ORDER. Plaintiff has filed a form Complaint for Review of Social Security Administration Decision. See Doc. #1 . That form requires plaintiff to provide the date on which the "Appeals Council denied Plaintiff's request for review or granted Plaintiff's request for review and issued a decision[.]" Id. at 2. Plaintiff states that the Appeals Council acted on "2/13/20[.]" Id. Plaintiff's complaint was filed in this Court on April 27, 2020. See id. at 1 (date stamp indicating filing occurred at 10:11 a.m. on "Apr. 27 2020" in New Haven). The Court is unable to determine, from the information provided, whether this action has been timely filed. Accordingly, on or before May 14, 2020, plaintiff shall file a copy of the Appeals Council's decision with the Court. Failure to comply with this Order may result in dismissal of this action. Filing 10 ORDER granting #2 Motion for Leave to Proceed in forma pauperis. Plaintiff has filed a motion seeking to proceed without payment of fees and costs, which motion includes a sworn statement as to plaintiff's current financial circumstances. See Doc. #2 . Plaintiff has completed the required information. Plaintiff asserts that she is unable to afford to pay fees and costs, having been unemployed since 2018 and having outstanding debt. See generally id. at 3-5. Plaintiff also states that she currently receives some government benefits, including "food stamps." Id. at 2. However, those benefits do not meet her monthly expenses. See id. at 2-3. At this stage, such allegations are sufficient to establish that plaintiff is unable to pay the ordinary filing fees required by the Court. See 28 U.S.C. 1915(a)(1).
