Filing 16 NOTICE OF INSUFFICIENCY. The plaintiff has filed a motion for leave to proceed in forma pauperis; in other words, he seeks to commence a federal lawsuit without pre-paying the filing fee. A federal law, 28 U.S.C. 1915, permits him to do so if, among other things, he files a certified copy of his inmate trust account statement for the six month period preceding his complaint. In this case, the plaintiff has now filed a trust account statement covering the six months preceding his complaint, but his application to proceed in forma pauperis is out of date. In particular, the certification of inmate account balance completed by a correctional counselor was signed April 8, 2022. The Clerk of the Court is respectfully directed to send the plaintiff a blank copy of the District of Connecticut inmate in forma pauperis application form at his address of record. The plaintiff is directed to fill out the form completely and accurately. In particular, the plaintiff is directed to complete page 5, which requires certification of his inmate account balance from a correctional counselor. The plaintiff is therefore directed to file with the Clerk of the Court an updated copy of the application form. If he does not do so by November 1, his case may be dismissed. If he does not do so by October 7, 2022, his case may be dismissed.Finally, the plaintiff is respectfully reminded that, in applying for in forma pauperis status, all of his entries on the application form must be true, correct, and complete.
