November 14, 2022 Opinion or Order Filing 10 ORDER. On October 14, 2022, the Court issued a Notice of Insufficiency informing Plaintiff that he must submit the correct in forma pauperis application form and a copy of his prison inmate account showing activity for the preceding six months before the Court would consider his motion. ECF No. 4. Plaintiff was advised that the case would be dismissed if he did not correct the deficiency by November 4, 2022. Id. As Plaintiff has not submitted the required documents or sought an extension of time within which to do so, his motion to proceed in forma pauperis ECF No. 2 is DENIED. Nor has Plaintiff paid the filing fee. Accordingly, the complaint is DISMISSED without prejudice. On or before December 29, 2022, Plaintiff may file a motion to reopen judgment accompanied by either the filing fee, or the correct application to proceed in forma pauperis and inmate account statement. October 14, 2022 Filing 4 NOTICE OF INSUFFICIENCY. The plaintiff has filed a motion for leave to proceed in forma pauperis; in other words, he seeks to commence a federal lawsuit without pre-paying the filing fee. A federal law, 28 U.S.C. 1915, permits him to do so if, among other things, he submits a financial affidavit showing that he is unable to pay, accompanied by a certified copy of his inmate trust account statement for the six month period preceding his complaint. To assist inmates in demonstrating inability to pay, the District of Connecticut has prepared a standard-form in forma pauperis application.In this case, the plaintiff submitted the wrong in forma pauperis application form. He also submitted a certified copy of his inmate trust account statement that only displays one month of activity. Six months is required. His application is therefore insufficient. The Clerk of the Court is respectfully directed to send the plaintiff a blank copy of the correct application form at his address of record. The plaintiff is directed to fill out the form completely and accurately, and to file it with the Clerk of the Court along with a certified copy of his inmate trust account statement covering at least the period from April 3, 2022, to October 3, 2022.
