Filing 9 ORDER: The Court has become aware that the Plaintiff is no longer housed at the MacDougall-Walker Correctional Institution as the Connecticut State Department of Correction website indicates he is now housed at the Robinson Correctional Institution. Therefore, it is not clear whether the Plaintiff received the Court's notice of insufficiency dated December 7, 2022. Accordingly, the clerk is instructed to send a copy of the Notice of Insufficiency [ECF No. 8] to the Plaintiff at the Robinson Correctional Institution. Failure to comply with the Notice of Insufficiency or this Order by January 19, 2023 may result in the dismissal of this action. The plaintiff is respectfully reminded that, anytime his address changes, he must promptly file a notice of change of address with the clerk of the court. Filing 8 NOTICE OF INSUFFICIENCY. The plaintiff has filed a motion for leave to proceed in forma pauperis - in other words, he has asked for permission to begin a civil lawsuit without pre-paying the $402 filing fee. A federal law, 28 U.S.C. 1915, permits him to do so if (among other things) he submits a financial affidavit showing that he is unable to pay, accompanied by a certified copy of his inmate trust account statement for the six month period preceding his complaint. To assist inmates in making this showing, the District of Connecticut has prepared a standard-form application. In this case, the plaintiff has submitted an application, but it is dated August 23, 2022, and is therefore out of date. His certified copy of his inmate trust account statement is also dated August 23, 2022, and is therefore out of date. The Clerk of the Court is respectfully directed to send him a blank application form at his address of record. The plaintiff is directed to fill out a new, up-to-date application and to file it with the Clerk of the Court along with a certified copy of his inmate trust account statement covering at least the period from June 6, 2022 to December 6, 2022.
