ORDER. Plaintiff's motion for leave to proceed in forma pauperis, ECF No. #4 , as supplemented, ECF No. #10 , is GRANTED to the extent Plaintiff is not required to prepay the $350.00 filing fee. Plaintiff is assessed an initial partial filing fee of $2.33 based on the monthly deposits in Plaintiffs inmate Trust Fund Account for the preceding six months as required by 28 U.S.C. 1915(b)(1)(A). Plaintiff has until May 20, 2019, to submit the initial partial filing fee. The Clerk of Court shall provide Plaintiff with a civil rights complaint form. Plaintiff has until May 24, 2019, in which to submit an amended complaint in compliance with the prior Court Orders. ORDER. Plaintiff's motion for reconsideration, ECF No. #8 , is GRANTED. The Report and Recommendation, ECF No. #5 , is VACATED. The Clerk of Court shall provide Plaintiff with another in forma pauperis application packet. Plaintiff shall have until May 10, 2019, in which to file a signed prisoner consent form and financial affidavit, supported by a copy of his resident account statement from the Florida State Hospital. ORDER. The Clerk of Court shall provide Plaintiff with an application packet for requesting leave to proceed in forma pauperis. On or before April 18, 2019, Plaintiff must file a response to this Order which explains the reason he is at the Florida State Hospital. On or before April 18, 2019, Plaintiff must file an in forma pauperis motion which is supported by copies of his resident account statement from the Hospital which reveals deposits and withdrawals in his account between September 1, 2018, and February 28, 2019. In the alternative, Plaintiff may pay the full amount of the filing fee ($400.00) if he desires to proceed with this case. The Clerk of Court shall return this file upon receipt of Plaintiff's compliance with this Order, or no later than April 18, 2019.
