November 21, 2022 Opinion or Order Filing 4 ORDER. The Plaintiff must file an amended complaint by December 27, 2022. The Clerk is DIRECTED to provide Davis with our form civil-rights complaint. We also DENY Davis's motion to proceed in forma pauperis ("IFP motion") [ECF No. #3 ]. Although the Plaintiff provided "an affidavit that includes a statement of all assets such prisoner possesses," he has failed to provide "a certified copy of the trust fund account statement (or institutional equivalent) for the prisoner for the 6-month period immediately preceding the filing of the complaint[.]" 28 U.S.C. 1915(a)(1)(2). The Plaintiff may refile his IFP motion by December 27, 2022. If he does so, however, he must include his six-month-account statement. If Davis fails to timely file his six-month-account statement, we'll DENY his IFP motion again. This Clerk's Office shall administratively CLOSE this case.
