Plaintiff's failure to file a completed Application to Proceed in District Court Without Prepaying Fees or Costs form within twenty-one (21) days of this Order will result in this Court's denial of her request to proceed in forma pauperis. The Clerk is DIRECTED to mail Plaintiff a copy of the Application to Proceed in District Court Without Prepaying Fees or Costs form with a copy of this Order.
