TEXT ORDER entered by Judge Colin Stirling Bruce on 2/22/2019. Plaintiff has filed a letter with the Court stating he was not allowed to participate in his merit review hearing. In addition, Plaintiff has filed a document with the Court which appears to repeat the claims in his complaint. The Court has not scheduled a merit review hearing in this case. Instead, the Court will review Plaintiff's allegations and provide a written order identifying Plaintiff's claims. In addition, Plaintiff should not file random documents with the Court to avoid confusion over the reason the document was filed. Plaintiff may instead file a motion, if appropriate, stating what specific relief he is requesting and why. Plaintiff will receive a copy of the merit review order when it is entered.(SAG, ilcd) |

TEXT ORDER granting #4 Petition to Proceed In Forma Pauperis. Pursuant to 28 U.S.C. Section 1915(b)(1), Plaintiff is assessed an initial partial filing fee of $5.67. The agency having custody of Plaintiff is directed to forward the initial partial filing fee from Plaintiff's account to the Clerk of Court. After payment of the initial partial filing fee (or immediately if no funds are available for that payment) the agency having custody of Plaintiff is directed to make monthly payments of 20 percent of the preceding month's income credited to Plaintiff's account to the Clerk of Court. Income includes all deposits from any source. The agency having custody of the plaintiff shall forward these payments each time Plaintiff's account exceeds $10, until the filing fee of $350 is paid in full. TEXT ORDER entered by Judge Colin Stirling Bruce on 1/24/2019. The Plaintiff has submitted a document claiming his Eighth and Fourteenth Amendment rights have been violated. #1 . The document was filed as a complaint pursuant to 42 U.S.C. 1983. See Bahler v Lopez, 2007 WL 1375924 (7th Cir. May 10, 2007). However, if Plaintiff wishes to pursue a lawsuit, he MUST either pay the $400 filing fee in full or file a motion to proceed in forma pauperis (IFP) on or before February 13, 2019. If Plaintiff fails to take any action on or before February 13, 2019, his case will be dismissed, but he will still be responsible for the $400 filing fee since a lawsuit was opened. In addition, if Plaintiff wishes to more clearly identify his claims and Defendants, he should use a standard complaint form to file an amended complaint. Any proposed amended complaint must also be filed on or before February 13, 2019. The Clerk of the Court is to provide Plaintiff with both a complaint form and an IFP form.
