Filing 6 WRITTEN Opinion entered by the Honorable John J. Tharp, Jr on 2/1/2013: Plaintiff's motion for leave to proceed in forma pauperis (Dkt. No. 3 ), is granted. The Court orders the trust fund officer at plaintiff's place of incarceration to deduct $10.51 from plaintiff's account for payment to the Clerk of Court as an initial partial filing fee, and to continue making monthly deductions in accordance with this order. The clerk shall send a copy of this order to the trus t fund officer at the Stateville Correctional Center. The Clerk is instructed to issue summonses for service on Defendant Ghosh, Zhang, Carter, Wexford Health Sources, Inc., Obaisi, and Downs. The Clerk is instructed to change defendant Wexford In c. to Wexford Health Sources, Inc. on the docket. Defendant Hardy is dismissed. The Clerk shall also send plaintiff a Magistrate Judge Consent Form, Amended Civil Rights Complaint Form, and Instructions for Submitting Documents along with a copy of this order. The United States Marshals Service is appointed to serve defendants Ghosh, Zhang, Carter, Wexford Health Sources, Inc., Obaisi, and Downs. Plaintiff's motions for appointment of counsel (Dkt.
