WRITTEN Opinion entered by the Honorable James B. Zagel on 6/10/2013: The plaintiff's motion for leave to proceed in forma pauperis 3 is granted. The court authorizes and orders the trust fund officer at the plaintiff's place of incarce ration to begin making monthly deductions in accordance with this order. The clerk is directed to send a copy of this order to the trust fund officer at the Cook County Jail. However, the complaint is summarily dismissed with prejudice on initial review for "fraud" on the court. The case is terminated. Any other motions pending are denied as moot.
