August 30, 2018 Opinion or Order Filing 5 ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is denied without prejudice. Plaintiff is directed to either (1) submit a renewed application that is certified by a trust fund officer and that includes a copy of Plaintiff's trust fund statements showing all transactions in his accounts for the six-month period immediately preceding the initiation of this lawsuit; or (2) pay the full statutory filing fee of $400.00. If Plaintiff was incarcerated at more than one facility during that time, he must obtain a certified trust fund account statement from each facility at which he was incarcerated. Failure to comply with this order by October 29, 2018, will result in dismissal of this case. Plaintiff also is advised that he must promptly submit a change-of-address notification if he is transferred to another facility or released. If Plaintiff fails to keep the Court informed of his address, this action will be subject to dismissal for failure to comply with a Court order and for failure to prosecute. Plaintiff's motion for attorney representation #4 , which does not reflect his efforts to obtain counsel on his own, is denied without prejudice to renewal should this action be allowed to proceed.
