Filing 5 ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is granted. The Court orders the trust fund officer at Plaintiff's place of incarceration to deduct $35.30 from Plaintiff's account for payment to the Clerk of Court as an initial partial payment of the filing fee, and to continue making monthly deductions in accordance with this order. The Clerk of Court is directed to send a copy of this order electronically to the Supervisor of the Inmate Trust Fund Accounts at the Cook County Jail. Summons, however, will not issue. Plaintiff's complaint #1 is dismissed without prejudice. Plaintiff is granted until October 26, 2018, to submit: (1) an amended complaint as detailed in this order, if she can do so consistent with Fed. R. Civ. P. 11; and (2) a completed USM-285 (Marshals service) form for each Defendant named in the amended complaint. Failure to submit the foregoing items by that date or to state a cognizable federal claim in any amended complaint will result in dismissal of this case for failure to state a claim. Plaintiff's motion for attorney representation #4 is denied without prejudice. Plaintiff is advised that she must promptly submit a change of address notification if she is released or transferred. If she fails to do so, this action will be subject to dismissal.
