ORDER: Plaintiff is directed to either file an application to proceed in forma pauperis on the enclosed form or pay the full $400 filing fee. Failure to comply with this order by December 31, 2018, will result in summary dismissal of this case. The Clerk of Court is directed to send Plaintiff a blank application to proceed in forma pauperis and a copy of this order. Plaintiff is instructed to file all future papers concerning this action with the Clerk of the United States District Court for the Northern District of Illinois in care of the Prisoner Correspondent. Plaintiff also must promptly submit a change-of-address notification if he is transferred to another facility or released.
