Filing 9 ORDER: Due to his transfer to the Cook County Jail, Plaintiff apparently did not receive the Court's May 2, 2019 order denying his application for leave to proceed in forma pauperis without prejudice. Plaintiff must either: (1) submit a renewed in forma pauperis application on the Court's required form that includes a certified copy of his trust fund statement showing activity in his account for the six months immediately preceding the filing of his complaint; or (2) pay the full statutory filing fee of $400.00. In light of his transfer, the Court extends the deadline to do so to July 20,2019. If Plaintiff fails to fully comply with this order by that date, the Court will summarily dismiss this case. The Clerk of Court is directed to send Plaintiff a blank application to proceed in forma pauperis, a copy of his complaint #1 , and a copy of the Court's May 2, 2019, order #6 .
