Upon review, plaintiff's complaint is dismissed without prejudice for failure to state a claim. The complaint does not include the basis for plaintiff's lawsuit. Based on the civil cover sheet, it appears that plaintiff may be seeking judicial review of a decision by the Social Security Administration. Pursuant to Northern District of Illinois Local Rule 8.1, "Where a compliant for judicial review is filed pursuant to 42 U.S.C. 405(g) and/or 42 U.S.C. 1383(c)(3)[:] (a) The complaint shall include the full Social Security number of the plaintiff, including that of a minor plaintiff not otherwise identified by his or her full name. If the plaintiff's application for Social Security benefits was filed on another person's wage-record, that person's Social Security number shall also be included in the complaint." If plaintiff wishes to proceed with this lawsuit, he must file an amended complaint that states the basis for his claim, and if he is seeking review of a social security decision, his complaint must include his full Social Security number. If plaintiff includes his social security number, he should write "To Be Filed Under Seal" on the top of his amended complaint, and the court will file the document under seal. Plaintiff is given until November 7, 2020 to file an amended complaint consistent with this order. If plaintiff does not file an amended complaint by that date, the case will be dismissed for failure to prosecute on the understanding that plaintiff has abandoned his claims. The Clerk of Court is directed to send Plaintiff Daniel Lawrence a blank application to proceed in forma pauperis and a copy of this order. Signed by the Honorable Martha M. Pacold on 10/7/2020.
