ORDER: The Court denies Plaintiff's application for leave to proceed in forma pauperis #3 without prejudice. To avoid summary dismissal, Plaintiff must file an amended and properly supported in forma pauperis application that accurately discloses his income starting twelve months before Plaintiff filed this action (June 7, 2020) and ending on the date of compliance with this order. Plaintiff must also include a trust fund certification completed by an authorized correctional official with supporting trust fund ledgers beginning on December 7, 2020, and ending on the date of compliance. Alternatively, Plaintiff may prepay the full $402 filing fee. If Plaintiff does not comply with this order on or before September 3, 2021, the Court will summarily dismiss this case.Plaintiff must promptly update his address if he is released or transferred to another facility. If Plaintiff fails to do so, this case will be subject to dismissal with no further warning. The Clerk is directed to send Plaintiff a copy of this order and an in forma pauperis form for incarcerated persons. See accompanying Statement for details. Signed by the Honorable John F.
