October 20, 2022 Opinion or Order Filing 6 ORDER: Plaintiff, who was a prisoner at the time he filed this lawsuit but who is no longer incarcerated, see #4 , filed this pro se civil rights lawsuit without submitting an application to proceed in forma pauperis (IFP) or paying the filing fee. Instead, he submitted a "motion providing information to the courts leave to file enclosed complaints request for IFP and access to law library and materials" #3 . The motion #3 is denied. If Plaintiff wishes to proceed further with lawsuit, he must resolve his fee status by submitting a properly completed IFP application (with supporting documentation) or pre-paying the $402 filing fee. Plaintiff is responsible for completing the IFP application and submitting the required information to the Court. Plaintiff is given until November 10, 2022 to resolve his fee status by submitting a properly completed IFP application (with supporting documentation) or pre-paying the $402 filing fee. The Clerk is instructed to terminate the document at docket no. #4 as a pending motion, as the docket has already been updated to reflect Plaintiff's current address. The Court notes that because Plaintiff was released since the filing of this case, see #4 , he must also include information about how he is currently supporting himself (new employment, public assistance, family, etc.) in any IFP application that he submits to this Court. Alternatively, Plaintiff may pay the full statutory filing fee of $402.00 by mailing a check or money order made payable to the Clerk of Court, which includes his name and the case number assigned to this case, to the Clerk of Court, United States District Court, 219 S. Dearborn Street, Chicago, Illinois 60604, attn: Cashier's Desk, 20th Floor. If Plaintiff fails to comply with this order by the aforementioned date, the Court will summarily dismiss this case. The Clerk is instructed to send Plaintiff a blank IFP application, along with a copy of this order.
