ORDER : Plaintiff's application for leave to proceed in forma pauperis #4 is provisionally granted for the reasons that will be explained. For the same reasons, the Court lacks the information to calculate an initial partial filing fee. For now, the trust fund officer at Plaintiff's place of incarceration is directed to make continuing monthly withdrawals in accordance with this order. To continue with this case in forma pauperis, Plaintiff must by February 24, 2020: (1) submit an application for leave to proceed in forma pauperis with a trust fund account statement obtained from an appropriate prison official that includes both a summary showing deposits and withdrawals over the previous six months, and a detailed transaction report of all deposits and debits from May 27, 2019, through the date he obtains it so that the Court can assess an initial partial filing fee; and (2) return and complete a separate USM-285 form for service for Defendants Lt. Burns, Lt. Kirby, and Officer Mayberry. Should he fail to comply with these directives, or if he fails to keep the Court apprised of his address, this case will be subject to dismissal. Once a determination has been made that Plaintiff has complied with this order, the Court will set a status hearing. [For further details see order]. Signed by the Honorable John Z.
