ORDER; The Court received $400.00 from Plaintiff shortly after he submitted his complaint in this matter, but effective December 1, 2020, the filing fee increased to $402.00. Plaintiff therefore must pay the remaining $2.00 he owes for the filing fee in this case. He should do so by November 25, 2021. Failure to pay the remaining $2.00 by this date will result in dismissal of this lawsuit for failure to comply with court orders. Further, Plaintiff's complaint #1 is dismissed without prejudice. Plaintiff is granted until November 25, 2021, to submit an amended complaint and a completed USM-285 (Marshals service) form for each Defendant named in the amended complaint. If he fails to comply, the Court will dismiss the case as frivolous and for failure to state a claim. Plaintiff is advised that he must promptly submit a change-of-address notification if he is released or transferred to another facility. If he does not keep the Clerk apprised of his correct address, this case may be dismissed. The Clerk is directed to send Plaintiff an amended civil rights complaint form and instructions, a blank USM-285 form, and a copy of this order. Plaintiff's motions to supplement the complaint #4 , to submit physical evidence and to conduct discovery #5 , for a "writ of protection" #6 , and to add defendants #7 are denied. By October 22, 2021, Plaintiff must either: (1) submit a completed application for leave to proceed in forma pauperis that includes a certified copy of the trust fund account statement (or institutional equivalent) for the six-month period immediately preceding the filing of the complaint and a completed certification from the appropriate official of each prison where Plaintiff is or was confined during the last six months; or (2) pay the full statutory filing fee of $402.
