| Filing 6 ENTRY - Plaintiff's Motion to Proceed In Forma Pauperis, [Dkt. #3 ], is GRANTED. 28 U.S.C. 1915(a). The form used by the plaintiff does not clearly state that the plaintiff is aware that she must report any spousal income. Because the plaintiff is represented, the Court has assumed that the plaintiff was made aware of her duty to report spousal income and that there was no spousal income to report. The Court has granted the motion based on those assumptions. If the plaintiff does have spousal income, the plaintiff is instructed to file an amended motion as soon as possible reporting the income and any household expenses. In the future, the plaintiffs representative shall make it clear in any motion to proceed in forma pauperis that her client is aware of the duty to report spousal income or the motion otherwise needs to explicitly state that there is no spousal income.
