February 19, 2019 Opinion or Order Filing 8 ORDER: 1. Delong's Motion to Proceed in forma pauperis DE #6 is GRANTED. 2. Because Delong has been granted pauper status in this proceeding, the $50 admin fee is waived. 3. The Clerk of Court shall open an account in Delong's name for receipt of the filing fee. The Clerk shall complete a Notice of Payment Form EDKY 525 and shall send the Notice and a copy of this Order to the warden of the institution in which Delong is confined. 4. Within 28 days, Delong's custodian shall deduct $23.33 from Delong's inmate account and remit that sum to the Clerk of Court as an initial partial filing fee. 5. For each month thereafter, if the amt in Delong's inmate account exceeds $10, his custodian shall deduct from Delong's inmate account an amount equal to 20% of his income for the preceding month and remit that sum to the Clerk of the Court. The custodian shall continue such mthly pymts until the entire $350.00 filing fee is paid. 6. The claims against Andrew Slay and GCDC officer Kevin Black are DISMISSED. 7. A Deputy Clerk shall prepare a "Service Packet" for each dft to include items herein. 8. The Deputy Clerk shall deliver the Service Packets to the USMS in Lexington, KY, and note the date of delivery in the docket. 9. The USMS shall make arrangments with the appropriate officials at the Greenup Co. Detention Center to personally serve nurse Belinda Moore, Dr. Rancie Hannah, and Jailer Mike Worthington.
