MEMORANDUM AND OPINION: Court ORDERS (1) Clerk to issue summons to Don Bottom, Warden, Blackburn Correctional Complex, to respond to Pla's 1 Complaint and 7 Motion for Supplement of New Evidence and Motion for Protective Order; (2) Clerk prepare copy of Complaint, Summons and Motion for Order of Protection and any required USM Forms 285 and hand deliver with copy of this Order to USM for service via certified mail or personal service, at option of USM; (3) w/i 40 days of date of this Order USM send Service Report to Lexington Clerk re status of service; (4) USM responsible for ensuring each def is personally served with process; (5) Clerk serve copy of Order on Kentucky Dept. of Corrections; (6) Pla shall keep Clerk of Court informed of his current mailing address; failure to notify Clerk of any address change may result in dismissal; (7) for further pleadings or documents Pla wishes to submit to Court, Pla shall serve upon Def, or attorney; Pla send original papers to be filed with Clerk of Court, with certificate of service; (8) If District Judge or Magistrate Judge receives any document not filed with Clerk or which has been filed but fails to include certificate of service, document will be disregarded by Court.
