March 11, 2020 Opinion or Order Filing 11 ORDER: 1) Garret's #9 Motion to Proceed In Forma Pauperis is GRANTED. 2) W/in 28 days from the date of this Order, Garret must pay $115.32 as an initial partial filing fee. 3) The Clerk of the Court shall open an account in Garrett's name for receipt of the filing fee. Clerk to complete a Notice of Payment Form w/ a) Garrett's name, b) his Inmate registration number, and c) this case number. A copy of this Order and Notice of Payment Form shall be serve to the Jailer/ Warden of the institution in which Garrett is confined and upon the Office of the General Counsel for the Dept of Corrections in Frankfort, KY. 4) Each month Garrett's custodian shall send the Clerk of the Court a payment in an amount equal to 20% of his income for the preceding month out of his inmate trust fund account, but only if the amount in the account exceeds $10.00. 5) Garrett must advise the Clerk's Office of any change in his current mailing address. 6) This matter stands submitted for initial screening.
