August 15, 2022 Opinion or Order Filing 4 ORDER: 1. Assibey-Mensah's current petition for a writ of habeas corpus [R. #1 ] is DENIED without prejudice, and this specific action is STRICKEN from the Court's docket. 2. That said, Assibey-Mensah may start a new civil action regarding the matter he raised. 3. The Clerk's Office shall send Assibey-Mensah the following blank forms so that he can initiate a new habeas action if he chooses to do so:a. Petition for a Writ of Habeas Corpus Under 2241 [E.D. Ky. 521 Form];b. Application to Proceed in District Court Without Prepaying Fees or Costs[AO 240 Form]; and c. Certificate of Inmate Account Form [E.D. Ky. 523 Form].4. If Assibey-Mensah wants to start a new case with this Court regarding the matter he raised in his initial submission, he may do so. However, Assibey-Mensah mustcomplete the E.D. Ky. 521 Form in its entirety, sign it, and file it with this Court. At the same time, Assibey-Mensah must also pay the $5.00 filing fee, complete and file both the AO 240 and E.D. Ky. 523 Forms, or at least submit a copy of acompleted BP-199 Form that he filed with prison officials requesting that the prisonwithdraw funds from his inmate account to pay the filing fee.
