Filing 11 MEMORANDUM AND OPINION & ORDER by Judge David J. Hale on 5/31/2017; IT IS ORDERED that Plaintiffs official-capacity claims and all individual-capacity claims against all Defendants except Plaintiffs claim against Defendants Smitty and Rodriguez in th eir individual capacities regarding Plaintiffs self-harm on December 21, 2016, are DISMISSED for failure to state a claim. IT IS FURTHER ORDERED that within 30 days from the entry date of this Memorandum Opinion and Order, Plaintiff may amend the co mplaint to provide additional facts of what he told Defendant Smitty and what self harm he did. The Clerk of Court is DIRECTED to place the instant case number and Amended on a § 1983 complaint form and send it to Plaintiff for his use should he wish to amend the complaint. The Court will conduct an initial review on the amended complaint pursuant to 28 U.S.C. § 1915A.
