MEMORANDUM AND ORDER--HEREBY ORDERED that plaintiff's motion to proceed in forma pauperis [Doc. # 2 ] is GRANTED. FURTHER ORDERED that the plaintiff shall pay an initial filing fee of $12.43 within thirty (30) days of the date of this Order . Plaintiff is instructed to make his remittance payable to Clerk, United States District Court, and to include upon it: (1) his name; (2) his prison registration number; (3) the case number; and (4) that theremittance is for an original proceeding. FURTHER ORDERED that plaintiff shall submit an amended complaint on a court-provided form no later than thirty (30) days from the date of this Memorandum and Order. FURTHER ORDERED that upon submission of the amended complaint, the Court shall again review this action pursuant to 28 U.S.C. § 1915. FURTHER ORDERED that the Court shall mail to plaintiff a copy of the Courts Prisoner Civil Rights Complaint form. FURTHER ORDERED that plaintiff's motion for appointment of counsel [Doc. # 4 ] is DENIED without prejudice. FURTHER ORDERED that plaintiff's motion for lien [Doc. # 5 ] is DENIED.(cc: finance/forms mailed to plaintiff) ( Response to Court due by 11/10/2012.). Signed by District Judge Jean C.
