Plaintiff is instructed to make his remittance payable to Clerk, United States District Court, and to include upon it: (1) his name; (2) his prison registration number; (3) the case number; and (4) the statement that the remittance is for an original proceeding. IT IS FURTHER ORDERED that plaintiffs motion for an extension of time to file a certified account statement (Docket No. #7 ) is DENIED as moot. IT IS FURTHER ORDERED that this case is DISMISSED without prejudice. A separate order of dismissal will be entered herewith. IT IS FURTHER ORDERED that plaintiffs Motion to Appoint Counsel (Docket No. #4 ) and his Amended Motion to Appoint Counsel (Docket No. #6 ) are DENIED as moot.
