MEMORANDUM AND ORDER: IT IS HEREBY ORDERED that Plaintiff's motion for leave to proceed informa pauperis [ECF No. #2 ] is GRANTED. IT IS FURTHER ORDERED that the Plaintiff shall pay an initial filing fee of $21.70 within thirty (30) days of the date of this Order. Plaintiff is instructed to make his remittance payable to "Clerk, United States District Court," and to include upon it: (1) his name; (2) his prison registration number; (3) the case number; and (4) that the remittance is for an original proceeding. IT IS FURTHER ORDERED that Plaintiff's motion for leave to file an amended complaint [ECF No. #8 ] is GRANTED in part and DENIED in part. Plaintiff shall file an amended complaint on a court-provided form within thirty (30) days of this Order. Plaintiffs motion is denied in all other respects. IT IS FURTHER ORDERED that the Clerk of Court shall mail to Plaintiff a blank Prisoner Civil Rights Complaint form. IT IS FINALLY ORDERED that upon submission of the amended complaint, the Court shall review this action pursuant to 28 U.S.C. 1915. Plaintiff's failure to timely comply with this Order could result in the dismissal of this action, without prejudice and without further notice. (Amended/Supplemental Pleadings due by 10/28/2022.)
