December 31, 2020	Filing 72 MEMORANDUM AND ORDER: IT IS HEREBY ORDERED that Plaintiff shall, within twenty days after the date of this Order, (1) pay the $505.00 in filing fees for his appeal or (2) file an affidavit complying with 28 U.S.C. § 1915(a)(1), as well as "a certified copy of [Plaintiff's] trust fund account statement (or institutional equivalent)... for the 6-month period immediately preceding the filing of [his]... notice of appeal, obtained from the appropriate official of each prison at which [Plaintiff] is or was confined," as required by 28 U.S.C. § 1915(a)(2).IT IS FURTHER ORDERED that any payment toward Plaintiff's appellate filing fees shall be made to the Clerk of this Court by a check or money order made payable to "Clerk of the United States District Court."IT IS FINALLY ORDERED that the Clerk of the Court shall mail a copy of this Order to Plaintiff at the: Eastern Reception Diagnostic and Correctional Center, 2727 Highway K, Bonne Terre, MO 63628.
