April 30, 2013 Opinion or Order Filing 8 MEMORANDUM AND ORDER - To obtain service of process on Defendants, Plaintiff must complete and return the summons forms that the Clerk of the court will provide. The Clerk of the court shall send two summons forms and two USM-285 forms to Plaintiff, together with a copy of this Memorandum and Order. Plaintiff shall, as soon as possible, complete the forms and send the completed forms back to the Clerk of the court. In the absence of the forms, service of process cannot occur. Upon receipt of th e completed forms, the Clerk of the court will sign the summons forms, to be forwarded with a copy of the Amended Complaint to the U.S. Marshal for service of process. The Marshal shall serve the summons and Amended Complaint without payment of co sts or fees. Service may be by certified mail pursuant to Federal Rule of Civil Procedure 4 and Nebraska law in the discretion of the Marshal. The Clerk of the court will copy the Amended Complaint, and Plaintiff does not need to do so. The Clerk of the court is directed to set a pro se case management deadline in this case with the following text: "August 29, 2013: Check for completion of service of summons." January 23, 2013 Opinion or Order Filing 6 MEMORANDUM AND ORDER - Plaintiff shall have until February 23, 2013, to amend his Complaint to specify the relief he seeks in accordance with Rule 8 of the Federal Rules of Civil Procedure. Any amended complaint must restate the allegations of Pla intiff's current Complaint and any new allegations (including additional allegations to support his ADA and ADEA claims). Failure to consolidate all claims into one document will result in the abandonment of claims. If Plaintiff fails to file an amended complaint that specifies the relief he seeks, this matter will be dismissed without prejudice and without further notice. Plaintiff should be mindful that the court had difficulty reading his handwritten allegations, and it cannot consider a llegations that are illegible. The Clerk of the court is directed to set a pro se case management deadline in this case using the following text: Check for amended complaint on February 23, 2013. Plaintiff shall keep the court informed of his current address at all times while this case is pending. Failure to do so may result in dismissal without further notice.
