Clerk shall send Plaintiff sufficient copies of first a mended complaint and USM-285 forms for each defendant (all forms delivered/mailed 9/26/12). Plaintiff shall have 20 days to furnish USM the required forms. Within 20 days after receiving from USM a copy of the USM-285 form showing whether se rvice has been accomplished, Plaintiff shall file notice with the court identifying which defendants were served and which were not served. ORDERED that, if specifically alleged in the amended complaint, the 8th Amendment deliberate indifference to medical needs claim shall proceed as to Ds Neighbors, Durante, Pierson, and Oksonhald . ORDERED that P's motion to exceed the page limit (# 8 ) for the amended complaint is GRANTED on the following basis. The amended complaint shall not exceed 30 pages. ORDERED that the Clerk shall send a blank section 1983 civil rights complaint form with instructions along with one copy of the original complaint to Plaintiff. (Docs mailed to P 1/5/2012.)
