Filing 7 ORDER: Plaintiff's #2 application to proceed in forma pauperis under 28 U.S.C. 1915 is granted. Plaintiff brings this #1 civil rights action alleging that defendants were deliberately indifferent to his serious medical condition. Pursuant to Valentin v. Dinkins, 121 F.3d 72 (2d Cir. 1997) (per curiam), the Court hereby directs the United States Attorney General's Office (the "Office") to identify the defendants allegedly involved in this case and to provide addresses for service of process by August 31, 2020. After the Office identifies and provides addresses for service of process for defendants, the Court shall direct the United States Marshals Service to serve the summons and complaint. Accordingly, plaintiff's #4 "motion for service of process by U.S. Marshal's Service," is denied. The parties shall advise the Court by signing the enclosed form whether they jointly consent to trial and decision on this case by me, a United States Magistrate Judge, pursuant to 28 U.S.C. 636(c). SO ORDERED by Magistrate Judge Lois Bloom, on July 21, 2020. C/mailed w/Attachments: #1 Consent to Trial and Decision by a United States Magistrate Judge. (The United States Attorney General's Office's Response due by 8/31/2020.) (Latka-Mucha, Wieslawa) |

Attached to the Notice is a blank copy of the consent form that should be filled out, signed and filed electronically only if all parties wish to consent. The form may also be accessed at the following link: #http://www.uscourts.gov/uscourts/FormsAndFees/Forms/AO085.pdf. You may withhold your consent without adverse substantive consequences.
