September 9, 2021 Opinion or Order ELECTRONIC ORDER re #6 Prisoner Authorization and #7 Notice of Deficient Filing. On July 14, 2021, Plaintiff Reynaldo Reyes ("Plaintiff") commenced this action by filing a Complaint without payment of the filing fee. (ECF No. #1 .) By Notice of Deficiency dated July 16, 2021, the Clerk of the Court instructed Plaintiff to either remit the filing fee or complete and return the enclosed in forma pauperis ("IFP") application and a form pursuant to the Prisoner Litigation Reform Act ("PLRA") within fourteen days. (ECF No. #2 .) On or around July 29, 2021, Plaintiff submitted an IFP application. (ECF No. #5 .) He also submitted an incomplete PLRA form, wherein he wrote "N/A" in every free space other than the case caption. (ECF No. #6 .) On July 30, 2021, the Court sent Plaintiff a Second Notice of Deficiency, explaining that Plaintiff did not answer the questions in the PLRA form, enclosed a new PLRA form, and cautioned that his failure to respond within fourteen days may lead to the dismissal of his Complaint for failure to prosecute. (ECF No. #7 .) To date, Plaintiff has not responded to the Notice of Deficiency or otherwise communicated with the Court. In an abundance of caution given Plaintiff's pro se status, the Court affords Plaintiff one final opportunity to complete the PLRA form. Accordingly, on or before September 27, 2021, Plaintiff SHALL complete, sign, and return the enclosed PLRA form. Plaintiff is ON NOTICE that in the event he fails to timely complete, sign, and return the PLRA form, the Court may dismiss this case without prejudice for failure to prosecute pursuant to Federal Rule of Civil Procedure 41(b). The Clerk of the Court is respectfully directed to mail a copy of this Electronic Order AND a PLRA form to the Plaintiff at his address of record. Ordered by Judge Joanna Seybert on 9/9/2021.
