IT IS ORDERED: (1) Plaintiff's motion to proceed in forma pauperis #1 is GRANTED and the tendered complaint shall be FILED; (2) Together with a copy of this Order, the Clerk of Court shall send to Plaintiff an additional copy of the complaint form used for complaints filed under 42 U.S.C. 2000e-5(f)(1), Form SD03; (3) As soon as practicable, but not later than 8/2/2019, Plaintiff shall supplement her Complaint with a statement clarifying whether she has received a Notice of her right to sue, and the date on which she received such Notice. In the alternative, she may provide the same information by filing an Amended Complaint using the form customarily used for pro se complaints filed under 42 U.S.C. 2000e-5(f)(1); (4) This Court will not direct service of Plaintiff's complaint on the Defendant in the absence of full screening under 28 U.S.C. 1915(e), which screening cannot be completed absent the supplementation or amendment of Plaintiffs complaint directed in paragraph 3; (5) Plaintiff shall keep the Court apprised of her current address; (6) Plaintiff is advised that any failure to comply with a court order, including but not limited to this Order, will result in a recommendation that her complaint be dismissed for failure to state a claim and/or for failure to prosecute.
