ORDER: Rodney T. Patterson, an inmate at the Northwest Correctional Complex in Tiptonville, Tennessee, filed a pro se civil rights complaint under 42 U.S.C. 1983 (Doc. No. #1 ) and an application to proceed in this Court without prepaying fees and costs (Doc. No. #2 ). Plaintiff's filings, however, suffer from a number of deficiencies. First, although Plaintiff signed a certificate of service regarding the complaint (Doc. No. #1 at 33), he did not sign the complaint itself (id. at 32). And second, Plaintiff did not sign the application to proceed in forma pauperis (Doc. No. #2 at 1), and the attached trust account statement is not certified by the custodian of Plaintiff's inmate trust account. Accordingly, the Clerk is directed to RETURN the complaint to Plaintiff. If Plaintiff seeks to have this Court consider his complaint, he MUST sign the complaint and either pay the full $400.00 filing fee, or submit a properly completed application to proceed in forma pauperis. The Clerk of Court is DIRECTED to mail Plaintiff a blank application to proceed in forma pauperis. Signed by Chief Judge Waverly D. Crenshaw, Jr on 2/4/2019.
