Filing 7 ORDER: From a review of Plaintiff's application to proceed in forma pauperis and supporting documents, it appears that Plaintiff lacks sufficient financial resources from which to pay the full filing fee in advance. Therefore, his application (Doc. No. #2 ) is GRANTED. Plaintiff Kenneth L. Peachman is hereby assessed the full civil filing fee. The Clerk of Court is DIRECTED send a copy of this Order to the administrator of inmate trust fund accounts at the Riverbend Maximum Security Institution to ensure that the custodian of Plaintiff's inmate trust account complies with that portion of 28 U.S.C. 1915 pertaining to the payment of the filing fee. Plaintiff's motion for the appointment of counsel (Doc. No. #3 ) is DENIED WITHOUT PREJUDICE. The Court has reviewed the complaint pursuant to the PLRA and finds that the complaint fails to state Section 1983 claims upon which relief can be granted against Warden Tony Mays and Medical Director Melissa Allen. Those Defendants therefore are DISMISSED. Plaintiff SHALL be permitted to amend his complaint, if he so desires, to identify the individual(s) directly responsible for denying medical treatment to Plaintiff for his dislocated shoulder. Plaintiff MUST submit an amended complaint within 30 days of entry of this Memorandum Opinion and accompanying Order. Otherwise, this case will be dismissed. The Clerk is DIRECTED to mail Plaintiff a copy of a blank civil rights complaint for prisoners and the Information Sheet entitled "How to Begin a Pro Se Civil Rights Action."
