Filing 5 Order and Notice of Deficiency: Antonio Hales must address the following deficiency: Plaintiff must file an Amended Complaint using the Courts civil rights form and Plaintiff must either pay the $402.00 filing and administrative fees or submit a request to proceed in forma pauperis on the District Courts Declaration form along with a certified inmate trust account statement. It is hereby ORDERED that, if he wishes to proceed with this case, Plaintiff shall cure the deficiencies as directed above by August 5, 2021. If Plaintiff wishes to have this case dismissed without the payment of the $402.00 filing and administrative fees or assessment of the $350.00 in forma pauperis filing fees under 28 U.S.C. 1915(b), he shall file a "Notice of Dismissal" within 14 days of the date of this order.
