The plaintiff may file an amended complaint, within thirty (30) days of this order, that states with sp ecificity (1) what he needs medical care for, (2) the name(s) of the individual(s) refusing to give him medical care, (2) why he needs a low bunk permit, (4) the name(s) of the individual(s) who refused his request for a low bunk permit, and (5) whet her or not his grievance has gone through the entire grievance process or at what stage it remains at this time. In short, plead more facts. If the plaintiff fails to file an amended complaint or follow the court's specific instructions, as out lined in this order, his case may be dismissed. The clerk is directed to provide the plaintiff with a blank complaint form to assist him. The clerk is directed to terminate Graham Correctional Center as a defendant. See written order.
