August 12, 2019 Opinion or Order Filing 5 ORDER: The Court grants Plaintiff DeShawn Johnson's application to proceed in forma pauperis #3 , waives the initial partial filing fee, and authorizes and orders the trust fund officer at Johnson's place of confinement to begin collecting monthly deductions from Johnson's account for payment to the Clerk toward the filing fee. The Clerk shall send a copy of this order electronically to: (1) the trust fund officer at the Cook County Jail; and (2) the Court's Fiscal Department. Summonses, however, will not issue as Johnson's complaint #1 is dismissed for failure to state a claim. By September 9, 2019, Johnson may file an amended complaint addressing the concerns in this order, as well as a completed USM-285 service form for each Defendant named in the amended complaint. If he fails to do so, the Court will summarily dismiss this case. Johnson's motion for attorney representation #4 is denied without prejudice, as it is too early to determine if counsel is appropriate. Johnson must promptly submit a change-of address notification if he is released or transferred to another facility. Failure to do will subject this action to dismissal. The Clerk is directed to send Johnson an amended civil rights complaint form, a blank USM-285 form and instructions, and a copy of this order. Signed by the Honorable Matthew F.
