ORDER - The Court cannot evaluate Mr. Waltons request to proceed in forma pauperis without a complete form. Therefore, the Court denies Mr. Waltons motion without prejudice. The Court asks the Clerk to please send Mr. Walton another copy of the in forma pauperis affidavit. Mr. Walton may complete that form, correcting the deficiencies described above, and file the completed form as a second motion to proceed in forma pauperis.
